Here is what we are going to make.

1. Make a new document 400x250 with a white background.
2. Use the type tool to type what you want to be 3D.
a. Use a darker color for the first layer.
Screen Shotb. Layer > Duplicate Layer
c. Now change the color of the font on the second layer to a lighter one.
Screen Shot3. Rasterize both of the layers by right clicking on the layer and selecting Rasterize Layer from the drop down menu.
a. Select the top layer and Edit > Transform > Perspective pull one of the bottom corners out to the side.
Screen Shotb. Select the bottom layer and Edit > Transform > Again.
4. Line both of the layers up in the center of the canvas.
a. Select the top layer and move it upward 15 pixels by pressing the Up Arrow Key 15 times.
Screen Shotb. Select the bottom layer and open the blending options.
c. Bevel and Emboss with the default settings.
Screen Shotd. Now, hold Alt and press the Up Arrow Key 14 times. This will duplicate the layer, one pixel above another, 14 times.
e. Select all of the 15 layers you just made (Discluding the lighter colored top layer) and Layer > Merge Linked.
5. Now select the top layer.
a. Open the blending options.
b. Stroke with these settings.
